Top Tips to Improve Chronic Illness Management with Your Patient
Chronic illness management can be hard but it doesn’t have to be. We have put together our top tips that can make a difference:
- Before you see the patient take the time to look at the clinical record before you call the patient in from the waiting room. This allows you to see where the patient is up to, start writing your progress notes and filling in what you can. This allows you to be present with the patient and not typing on the computer.
- Book the next appointment. This is one of the simplest processes a practice can implement reminders are costly ineffective and delay management. Explain to your patient why you need to book the next appointment. They commit in person and by booking the next appointment with reception, you increase their chances of attending the appointment.
- Make your patient goals smaller. Goals setting needs to be manageable for you and your patient. If you ask them to complete too many tasks before the next visit you increase the chance of them not coming. One or two tasks is maximum. Our goal is to see our patient regularly and provide long term care.
