Our Purpose

Our purpose at CDM Plus is to help consumers, carers, and health professionals to increase self-management for those at risk of or living with chronic conditions.

CDM Plus offers products and services for consumers and carers that can help improve their wellbeing by prompting them to identify disease risk factors, recognise behaviour patterns, set personal goals and establish healthier habits.  Consumers can stay accountable, motivated and make progress towards their goals on their own or with the support of our dedicated Health and Wellness coaching services.

We provide training, products, and resources for Health Professionals in Primary Healthcare, Aged Care, Community and Disability to empower them to deliver long term quality care and coordination.  CDM Plus delivers training throughout Australia onsite and remotely.

We understand that all organisations operate a bit differently and can work with you to create and deliver a custom training package.

Our Founder and CEO – Jane Calligeros

Jane founded CDM Plus in 2014 with the goal of helping improve prevention, detection and management of patients at risk or with established Chronic Disease.  Her nursing career began in 2000 where she gained experience in Acute Care in both Australia and the UK with unique experience working across Community, Occupational and Corporate Health before moving to Primary Healthcare. Her previous experience in Community and Occupational Health saw her coordinating and implementing off-site services for patients and corporate clients.

Jane followed her passion to Primary Healthcare in 2006 where she has worked as a Chronic Disease Nurse in both General Practices and Aboriginal Medical Services.

She has completed Post Graduate qualifications in Chronic Disease Management and further training in Health and Wellness Coaching.  Her knowledge of clinical software, funding mechanisms, Medicare Item Number requirements and development of system-based processes to improve Chronic Disease Management make her a valuable resource for Primary Healthcare and continue to make CDM Plus leaders in their field.

Gloria has been with CDM Plus for over 9 years and has demonstrated experience in Business and Brand Development.  Gloria has been with CDM Plus since its inception and works with our new and existing customers building long term and successful relationships with key stakeholders to help CDM Plus deliver custom training packages and projects throughout Australia.  Her understanding and knowledge of CDM Plus’s products and services helps customers, organisations and governments find the solutions and training they need.
