Getting Started with Integrated Team Care (ITC)

Our Getting Started short courses are delivered as a single 2.5-hour individual remote training session with one of our experienced CDM Plus Trainers.  Our sessions will use your current clinical software, including Best Practice, Medical Director, Communicare, and Zedmed, to increase understanding of care coordination and improve health outcomes for Aboriginal and Torres Strait Islander peoples.

Getting Started with ITC is suitable for staff involved in the ITC program, including Aboriginal Health Workers, Aboriginal Health Practitioners, and Nurses, who are looking to improve Complex Care service delivery with a specific focus on the ITC Program.

Course Objectives

  • Identify Integrated Team Care (ITC) program roles and workflows
  • List Local, State/Territory, and Federal funding currently available
  • Increase confidence to apply knowledge and implement Integrated Team Care (ITC) services in your organisation
