Getting Started with Chronic Disease Management – Individual

Our Getting Started short courses are delivered as a single 2.5-hour remote training session for Individuals with one of our experienced CDM Plus Trainers. Our session will use your current clinical software, including Best Practice, Medical Director, and Communicare to increase confidence and clinical skills in Chronic Disease Management activities. Hard Copy training materials will be provided prior to your session.

Getting Started with CDM is suitable for Nurses, Aboriginal Health Workers and Practitioners, Nurse Practitioners, Practice Managers, and GPs looking to improve Chronic Disease Management service delivery.

Course Objectives

• Identify the different Chronic Disease Management settings and practice team members
• Differentiate between Medicare Chronic Disease Management Items
• Identify eligible patients for Chronic Disease Management activities
• Increase confidence to perform Chronic Disease Management activities using your clinical software

• Chronic Disease Management Process and Team Roles
• Medicare Item numbers and navigating the MBS Online Services
• Home Medicines Review (HMR)
• Templates and Text Shortcuts
• Care Plans and Reviews (GPMP/TCAs)
• Care Plan demonstration using your clinical software
