Getting Started with Chronic Conditions – Individual

Our Getting Started short courses are delivered as a single 2.5-hour remote training session for Individuals with one of our experienced CDM Plus Trainers. Our session will use your current clinical software, including Best Practice, Medical Director, and Communicare to increase confidence and clinical skills in the prevention, early detection, and management of common Chronic Conditions.  Hard Copy training materials will be provided prior to your session.

Getting Started with Chronic Conditions is suitable for Nurses, Aboriginal Health Workers and Practitioners, Nurse Practitioners, and GPs looking to improve Chronic Disease Management service delivery.

Course Objectives

  • Identify risk factors for common chronic conditions
  • Identify MBS Item Numbers for Chronic Disease Management Detection and Management activities
  • Identify relevant lifestyle modifications and targets for each chronic condition

What we will cover in the training session:

  • Prevention, Detection and Management of chronic conditions such as Asthma/COPD, Diabetes, Cancer, Osteoporosis, Cardiovascular Disease and Chronic Kidney Disease
  • MBS Item numbers for common detection and management activities
  • Clinical Guidelines for primary care
  • Case Studies
