• My Care Plan – Health & Wellness is a hard-copy resource that helps consumers improve their wellbeing by identifying disease risk factors, recognising behaviour patterns, setting personal goals, and establishing healthier habits. My Health & Wellness allows consumers to stay accountable, motivated, and make progress towards their goals.
• My Care Plan – Chronic Care is an A4 hard copy resource that is designed to help patients self-manage their chronic conditions. Consumers, family members, or carers can also use this to keep track of medical appointments, health history, observations, medicines, and goals. It also includes customisable escalation plan.
• My Care Plan – Complex Care is an A4 hard copy, customisable resource that helps consumers, family members, and carers self-manage multiple complex chronic conditions. The folder can be personalised to meet consumer’s health needs and goals, improve communication between patients, carers, and health professionals, track and organises appointments and test results, document health history and medicines, record observations and goals and establish a Personalised Emergency Evacuation Plan (PEEP) to ensure you have the necessary personal and health information easily accessible in an emergency.
