Getting Started with Health Assessments – Individual

Our Getting Started short courses are delivered as a single 2.5-hour remote training session for Individuals with one of our experienced CDM Plus Trainers. Our session will use your current clinical software, including Best Practice, Medical Director, and Communicare to increase confidence and clinical skills in completing a Health Assessment. Hard Copy training materials will be provided prior to your session.

Getting Started with Health Assessments is suitable for Nurses, Aboriginal Health Workers and Practitioners, Nurse Practitioners, and GPs looking to improve Chronic Disease Management service delivery.

Course Objectives

• Identify the different types of Health Assessments in Primary Health Care
• Differentiate between Medicare Chronic Disease Management Items for Health Assessments
• Identify eligible patients for Health Assessments
• Increase confidence to perform a Health Assessment using your clinical software
