My Care Plan – Chronic Care is a hard copy resource designed to help people self-manage their chronic conditions like Cardiovascular Disease, Diabetes, Asthma, COPD, Chronic Kidney Disease, and Cancer. Self-management can help you or your loved one lead a happier and healthier life by giving them the knowledge, skills, and confidence to effectively manage their chronic conditions.
Self-management is your ability to participate and be in control of your own health. Self-management includes activities such as following your treatment plan, taking your medicines as prescribed, monitoring your observations and symptoms, practicing self-care techniques, setting goals, and making lifestyle changes.

Our My Care Plan – Chronic Care includes:
- My Care Plan – Chronic Care Booklet is an A4, 80-page, 12-month customisable health planner that includes set-up instructions to help you self-manage your chronic condition. The booklet allows you to record your Care Team, Health History, Family History, Medicines, and Immunisations. Your Self-Management sections allow you to map your Annual Plan, Monthly Plan, and Set Goals. Your Monthly Plan lets you set Goals, Tasks, and keep consistent with a daily Habit Tracker. Depending on your chronic condition you may need to keep track of certain Observations like your Blood Pressure. The Observations double page lets you record Blood Pressure, Heart Rate, Temperature, Weight, Activities, Active Minutes, Blood Glucose Levels, Oxygen Saturation, Peak Flow, and Fluid Intake. There is a Notes double page each month so you can keep a timeline of your health and visits to other providers. Your Escalation Protocol is a traffic light system, so you know when you are on Target (Green), when you need to contact your GP (Yellow) or when you need to Call an Ambulance or go to Hospital (Red).
