Meet some of the Team

At CDM Plus our purpose is to help practices better manage patients with Chronic Disease and promote activities that can prevent, delay and detect Chronic Disease.

We provide training and resources for GPs, Nurses, AHWs and Reception to enable them to deliver quality long-term chronic disease management and care co-ordination.

We understand that all practices operate a bit differently and offer a variety of training options to give the best results to our clients and patients.


Workshops – Group Training – Online Modules – Resources


CDM Plus Workshops are delivered all over Australia and our Group Training can be delivered onsite and remotely.  If you prefer something online then our Online Modules or Remote Training are available.

If you need your whole practice team on the same page then our Chronic Disease Management resources can help and available for Individuals or Practices.


Jane Calligeros – Owner / Lead Consultant

Jane has 18 years of Nursing experience; the last seven years working in General Practice. Jane began her nursing career in 2000 after completing her Bachelor of Nursing in Newcastle.

Jane followed her passion of Primary Health Care in 2006 including Post Graduate qualifications in Chronic Disease Management in 2015.
Jane’s goals relating to improving management, early detection and prevention strategies for patients at risk or with established Chronic Disease have been grounded and improved whilst working in General Practice Services, both large and small, including Indigenous Health services.

Jane was nominated in 2015 and 2017 for an APNA Best Practice Award in Chronic Disease Management for her accomplishments in Indigenous Health.
Her previous experience in Community and Occupational Health saw her coordinating and implementing off-site services for patients and corporate clients.

Jane’s unique nursing background and knowledge of funding mechanisms, Medicare Item Number requirements is a valuable resource for General Practice and continues to make CDM Plus leaders in their field.

Gloria Stewart – Manager

Experienced Manager with a demonstrated history of working in Business as a consultant in the medical practice industry. Skilled in Management, Customer Service, Strategic Planning, Sales Management, and Leadership. Gloria runs the day to day operations of CDM Plus and Brand Development and Co-ordinates our Admin Team.

Katina Rogers – Social Media and Events Manager

Is an experienced business owner and has a strong background in Social Media and Events. She co-ordinates our SME Team for our Virtual and Face to Face Workshops and Events as well as our CDM Social Pages. Her efficient and easy-going nature allows staff to feel at ease as they are undertaking new tasks and processes.
