Getting Started with Integrated Team Care (ITC) – Small Group

Our Getting Started short courses are delivered as a single 2.5-hour remote training session for Small Groups (up to 3 people) with one of our experienced CDM Plus Trainers. Our session will use your current clinical software, including Best Practice, Medical Director, Communicare to increase confidence and clinical skills to increase understanding of care coordination and improve health outcomes for Aboriginal and Torres Strait Islander peoples.  Digital training materials will be provided prior to your session.

Getting Started with Integrated Team Care is suitable for staff involved in the ITC program, including Aboriginal Health Workers, Aboriginal Health Practitioners, and Nurses, who are looking to improve Complex Care service delivery with a specific focus on the ITC Program.

Course Objectives

  • Identify Integrated Team Care (ITC) program roles and workflows
  • List Local, State/Territory, and Federal funding currently available
  • Increase confidence to apply knowledge and implement Integrated Team Care (ITC) services in your organisation
