What observations do I include in a Care Plan or Review?
Record observations that are relevant to the patient’s age and conditions:
- Height/Weight/BMI/Waist
- Blood Pressure and Heart Rate
- Blood Glucose Level
- Visual Acuity
- ECG (Annual Chronic Disease)
- Peak Flow (Respiratory Disease)
- Spirometry (Annual patients >8yrs old with respiratory disease)
- Oxygen Saturation (Respiratory Disease)
- Respiratory Rate (Respiratory Disease)
- Urinalysis (Chronic Disease)
