What are Care Plans & Reviews?
Care Plans and Reviews are some of the most common Chronic Disease Management activities performed by Nurses, AHWs, and GPs.
Care Plan is the term used to describe the appointment type and document created that contains the patient’s:
- Conditions
- Needs & Goals
- Treatments & Services
Care Plan Reviews are used to describe the appointment type and document that “Reviews” the Care Plan previously created.
