Reception staff play an important part in Chronic Disease Management. 

All Processes begin and end with Reception (Booking & Confirming Appointments, Checking Patient Eligibility and Updating Patient Details).

They often know your patients as well as you do!

Things to Consider:

  • Checking eligibility– All patients need to have their eligibility checked before completing a Care Plan or Review. Reception can assist by completing patient eligibility checks and documenting their eligibility in the appointment notes. Staff can use the MBS Online Checker via HPOS or contact Medicare.
  • Confirming Appointment– All CDM Appointments need to be confirmed the day before. Reception can do this via an SMS reminder or by phone.
  • Updating Patient Details– Reception can assist by updating patient details on arrival.
  • Next Appointment– Booking the Next Appointment for a Care Plan or Review can help increase patient engagement and attendance rates. It also allows your practice to plan staff resources.
