Nurse/Aboriginal Health Worker

Nurses and Aboriginal Health Workers assist the GPs in coordinating and delivering long-term care for patients with chronic diseases by preparing Care Plans and Reviews with patients.

They also play an important role in prevention and early intervention through screening activities such as Health Assessments.

Things to Consider:

  • Process– For practices with more than one Nurse/AHW, the Care Plan or Review process needs to be consistent. We will give you some tips to get started.
  • Text Shortcuts (also known as autofills)- Care Plans & Care Plan Reviews must align with Medicare requirements. A way to achieve this is to set up autofills for your progress notes. Autofills act like a checklist to ensure you include all the required information.
  • Templates– Setting up templates for Care Plans and Reviews can improve efficiencies in Chronic Disease Management delivery.
  • Time– How long do you need to prepare a Care Plan or Review? It is helpful to decide this with your team and then set up some appointment types. Appointment times are something to review at your team meetings.
