Memberships for individuals and teams in Primary Care

At CDM Plus we understand that every health professional and practice team have different learning needs when it comes to chronic disease management. Our membership model is built to support healthcare professionals and practice teams involved in chronic disease management including clinical and admin staff. Whether you are an individual practitioner or part of a team, each membership tier gives you the tools, training, and resources to improve patient care, streamline your practice, and stay up to date with the latest in chronic disease management.
We have four membership tiers to offer the right support for wherever you are in your journey:
  • Starter Membership is perfect for those individual healthcare professionals looking to access the essentials in chronic disease management including templates, digital resources, and online modules for the beginner and advanced.  You will also be invited to attend our exclusive member webinars.
  • Practice Membership is perfect for small practice teams looking for high performance and long-term solutions in chronic disease management.  This membership can have up to 15 users and includes a remote small group training session from our Getting Started Series with one of our trainers and our new Watch & Learn Series.  All our practice memberships include our starter essentials including templates, digital resources, online modules, and exclusive webinars to help set up your team for success. Our Practice Membership also includes a bonus Health Promotion Box of your choice to help kickstart your prevention and early detection activities in 2025.
  • Premium Membership is perfect for medium practice teams looking for high performance and long-term solutions in chronic disease management.  This membership can have up to 25 users and includes a remote small group training session from our Getting Started Series with one of our trainers, our new Watch & Learn Series, and a face to face 2-day Workshop Bundle ticket.  All our practice memberships include our starter essentials including templates, digital resources, online modules, and exclusive webinars to help set up your team for success. Our Premium Membership also includes two Health Promotion Boxes of your choice to help kickstart your prevention and early detection activities in 2025.
  • VIP Membership is perfect for large practice teams looking for high performance and long-term solutions in chronic disease management.  This membership can have up to 50 users and includes a remote small group training session from our Getting Started Series with one of our trainers, our new Watch & Learn Series, and a face to face 2-day Workshop Bundle ticket.  All our practice memberships include our starter essentials including templates, digital resources, online modules, and exclusive webinars to help set up your team for success. Our VIP Membership also includes our Health Promotion Program which includes four Health Promotion Boxes of your choice to help kickstart your prevention and early detection activities in 2025.

What is a CDM Plus Membership?

Our membership model is designed to support healthcare professionals and primary care teams involved in chronic disease management including clinical and admin staff.

We have Starter available for our single user health professionals.  Primary Care Teams can choose from Practice, Premium, and VIP Memberships. 

Our primary care team memberships are per practice as they include a small group training (Practice, Premium and VIP) and a 2-day Workshop Bundle (Premium and VIP). Please contact us on 1800 549 769 or email [email protected] to discuss your needs.

Yes, you have 12 months from date of purchase to use the small group training session
and workshop bundle ticket.

Yes, you have 12 months from date of purchase to use the 2-day Workshop Bundle.

CDM Plus Memberships are 12 months from the date of purchase.  Your membership will auto-renew in 12 months.  If you do not wish to renew your membership you can cancel your membership before the renewal date or change to a different membership tier.
