Care Plan Process using the CDM Manual

Our Chronic Disease Management Manual was developed to support clinical and admin staff in Primary Healthcare by simplifying Chronic Disease Management processes into a series of steps.

These steps are to be used as a guide and processes may vary from practice to practice.

Each chapter in the manual contains icons for patients and practice team members to identify the different steps to be completed by staff and identify eligible patients for Chronic Disease Management activities. The beginning of every chapter contains Medicare information to help you identify eligible patients.
